You’ll be given two forms by your BLO. Both must be filled carefully.

Case 1: Your Name Is Already in the 2002 Voter List
If your name is listed in the 2002 voter list, the process is super easy:
- Write your name in the voter’s name column.
- Fill in your EPIC number (Voter ID number).
- Add your district, state, and assembly constituency name.
- Then note your assembly constituency number, part number, and serial number you’ll find these in the 2002 voter list.
Once you fill this section:
- Sign the form at the bottom.
- Attach a photocopy of the page from the 2002 list where your name appears.
- No other documents are needed.

Case 2: Your Name Is NOT in the 2002 Voter List (But Your Parents’ Name Is)
